The Jersey Health and Care Partnership Group

The Jersey Health and Care Partnership Group (ā€˜the Groupā€™) is a new group that is being established as part of the Jersey Care Model (JCM) Programme. The overarching principles of the JCM are to transform health and social care in Jersey and to improve Islandersā€™ physical and mental health.

It adopts a person-centred approach where care is safe, accessible, financially sustainable and delivered where Islanders need it most; whether thatā€™s in the personā€™s home, in the community or in hospital. It helps to improve Islandersā€™ physical and mental health by promoting better self-care, utilising advances in technology, and delivering more services outside of a hospital setting in partnership with local health and care community providers.

The Group plays a key role in extending and strengthening partnership working within the Island to deliver new, integrated health and care pathways for Islanders.

The Group will act in an advisory role for the various projects in the Jersey Care Model programme with the aim to exchange improvement and integration ideas for health and care services across the Island and will interact closely with the two additional advisory groups, the Clinical and Professional Advisory Forum and the User Experience Group.
